Default Performance rating scales

I am implementing an online system to manage performance, development & succession. We have a current discussion about the best descriptors for our 5 scales performance rating-we currently use:
1=Unsatisfactory
2=Inadequate achievement
3=Achieves requirement
4=Exceeds requirement
5=Exceptional
I would be interested to know your descriptors?

(1) No opportunity to display
(2) Needs improvement
(3) Meets the standard required
(4) Above the standard required
(5) Regularly exceeds the standard required
